This report examines program implementation and progress from November 1, 2014 through December 31, 2021. Using archival, survey, and qualitative data, the evaluation team assessed housing retention rates as well as client characteristics and risk factors that contribute to differential program outcomes. Additionally, the team examined the impact of trauma and housing location on client wellbeing and housing outcomes. Given the increased reliance on Telehealth during COVID-19, evaluators also accessed client technology access. Finally, the team used geographical information system mapping analysis to develop an app to aid in ongoing service provision and future housing navigation. The report presents these data and concludes with recommendations for the program & funders and with next steps for the evaluation.
